Case management is the process whereby a professional supports client to develop a plan that coordinates and integrates the support services that the client needs to optimize overall healthcare, employment, OW, social, housing, education, and psychosocial possible goals and outcomes.
With the clients/and or their family members’ consents, the Case Managers collaborate with physicians, nurses, social workers, housing, employment agencies, Educators, and other healthcare professionals to develop comprehensive care plans, coordinate with Social Service agencies where necessary on behalf of the client, and ensure that they access necessary services and social support.
Our Case Managers serve as our clients advocate to support, guide, and coordinate care for our clients and families, and caregivers as they navigate their overall needs through screening, assessment, stratifying risk, planning, implementing, follow up, transitioning, communicating of post transition and evaluating outcomes. They also assist clients to understand their options concerning the specific situation they are dealing with at the time.
